We serve communities where close to half of residents are from minority ethnic backgrounds, including one of the largest Black populations in the country. Our job is not only to treat illness, but to reach the people good care too often misses, and to help close the gaps that hold our neighbourhoods back.

Closing the gap

We use proactive outreach and technology to find and help the patients who are hardest to reach, where the need is greatest.
Finding cancer earlier
Our proactive screening outreach, including prostate cancer screening, helps find cancers sooner in communities where they are too often caught late.
Transforming diabetes care
By systematically reaching patients who are hardest to contact, we raised completed diabetes care reviews from under a third to around two thirds in a few months.
Care that comes to you
When housebound patients risked missing out on COVID and flu vaccination, our teams stepped in to vaccinate them at home across Lewisham.
Reaching the seldom heard
Out-of-area registration is welcomed at several practices, and around 80 to 85% of patient contacts are still by phone or in person, so digitally excluded patients are never shut out.

Language barriers, shift work, insecure housing and digital exclusion mean traditional GP recall misses the very patients with the greatest need. Our proactive outreach, by text, app, email and phone, is built to reach everyone with a care gap, whoever and wherever they are.”

Connecting you to local support
Good health is about more than medicine. Our care coordinators connect patients to a wide network of more than 400 local and national support services, covering mental health, housing and debt advice, weight management, stop-smoking support and befriending. We work alongside local NHS trusts and councils, including Guy’s and St Thomas’, King’s College Hospital and South London and Maudsley, to join up your care.
