We look for the patients others miss

Traditional general practice waits for patients to get in touch. We do that too, but we also work the other way around. Using our records, we find patients who are due a review, or a check they may have missed, and we invite them, by text, the NHS App, email or a phone call.

This matters most for long-term conditions like diabetes and high blood pressure, where regular checks prevent serious problems later. When we focused on our patients living with diabetes, we found and invited everyone who was overdue, and within a few months far more were getting the reviews that keep them well. In one recent six-month period, this proactive approach created around twelve thousand extra check-ups across our practices.

No one should miss out on care because a letter did not arrive, or a call came in the middle of a work shift. Reaching out, in more than one way, helps us care for everyone, not just the people who find it easiest to reach us.
